I'm watching the audio commentary on First Blood by Sly right now, and i think it is really great!
He is fantastic at explaining background stories about the film and it's cast.

As far as I can tell you, the pre-production of FIRST BLOOD was very very troubled.
Did they film any scene featuring Kirk Douglas as Trautman? And how on hell did they cut all those scenes?

Douglas was rejected because he didn't like the way the Rambo character was written.

Kirk Douglas wanted Rambo to die I think - as per the original book. He and Sly and a few of the higher-ups had creative differences. They didn't want their script rewritten. They said, 'We want you for the part - here's the script - as is - it's your call.' Next thing you know he's on a plane back home. This is featured in the extras on the Trilogy that came out on DVD a while back.

I'm glad crenna ended up being trautman instead of douglas, I just can't see the role played by anyone else.

Kirk Douglas (the original choice for Trautman) insisted that Rambo should have died in the end, that's why they (apparently) fired him. Or so they say. But WAIT! They REALLY filmed the Rambo death scene as it would have beeen the ONLY POSSIBLE ENDING to 'FIRST BLOOD'. When at test screenings the audience reaction was TERRIBLE (people angry and saddened about Rambo's death), they filmed the positive ending scene we all know and love. So what? Why did they fire Douglas if Rambo was MEANT to die from the beginning?

Right from the beginning they showed Rambo as a man that didn't want trouble.
That's why the audience disliked the death of him.
If Rambo was brutal, (as in the book), then they probably wouldn't have protested.
